Domperidone
Level 3: This medication poses a specific safety risk that is likely to be harmful, it should not be taken.
Domperidone is a peripherally selective dopamine receptor antagonist used outside the United States as an anti-emetic and to boost breast milk production in breastfeeding women (both cis and trans). This medication achieves its effects through high blood prolactin levels. This medication has no evidence of its use in gender-affirming care and has significant risks (see "Safety and Effects").
Dosing and Monitoring
Due to having no official use in gender-affirming care, there is no basis for dosing recommendations. Hyperprolactinemia must be monitored with this medication (Madhusoodanan Subramoniam, 2010) (for more information see "Safety and Effects").
Safety and Effects
Domperidone and Hyperprolactinemia
The method of action for domperidone is hyperprolactinemia; it is something the medication directly causes (Madhusoodanan Subramoniam, 2010). Hyperprolactinemia can cause several problems including cardiovascular risk, it also has little to no benefit on gender-affirming care (Haring Robin, 2014)](https://www.doi.org/10.1093/eurheartj/ehs233).
Domperidone and Cardiovascular Problems
Domperidone by itself shows a significant risk of heart arrhythmia a risk that should not be taken lightly by anyone (Ou Linda, 2021). It is unknown if these cardiovascular problems are related to hyperprolactinemia or if they are a problem caused by domperidone itself, thus monitoring for hyperprolactinemia does not completely mitigate this risk.
